| Description: | Responsible for providing primary care as well as Behavioral Health services to members; assesses members’ health needs and develops associated treatment plans; coordinates members’ care with providers from various specialties while focusing on the needs of the “whole person”; integrates Evidence Based Practices (EBP’s) into clinical programming, champions preventive care and healthy lifestyle changes; and provides educational training on medical conditions, medication adherence, condition management, etc. |
| Qualifications: | Requires knowledge of Behavioral Health Treatment modalities; Bachelor’s or AA in Nursing with two years of relevant work experience; valid Maryland Nursing License; valid driver’s license; and safe driving record. |
