| Description: | This is a direct service position requiring knowledge of serious mental illness and specific knowledge and skill in rehabilitation and treatment for persons. Positions requires generalist skills, case management, ADL's, as well as specific knowledge of Evidence Based Practice's related to persons with serious mental illness. This is a hourly position. |
| Duties: | Conduct visits to various agencies, shelters and hospitals to engage clients for the ACT Program., Coordinate client care and collaborate with other social and community agencies, advocating for clients; refer clients to other treatment facilities/community resources as appropriate. Participate in the provision of rehabiliation services in coordination with the team. Become a member of the ACT philosophy and follow and maintain high fidelity of the ACT EBP model. |
| Qualifications: | Minimum of a high school diploma; AA degree in related field preferred.
Current and/or previous involvement as a client of mental health services.
Minimum of 2 years positively referenced relevant work experience
Possession or use of a four passenger car to be used to transport clients and a copy of a valid certificate of automobile insurance
Valid driver's license and safe driving record.
